Community Health Needs Assessment/Community Health Survey

Healthy Mendocino has partnered with Mendocino County Health and Human Services Agency, Ukiah Valley Medical Center, Frank R. Howard Memorial Hospital, the Alliance for Rural Community Health, and North Coast Opportunities to conduct a county-wide community health needs assessment to identify the specific needs and challenges of the community. As part of the project, the coalition developed an anonymous, community-wide survey that Mendocino County residents are asked to complete by September 30.

The survey asks about a range of issues including gaps in health care services, general health and insurance information, housing barriers, and more. It also asks respondents to identify the most pressing issues facing Mendocino County families.The survey is also available in print form, in English and Spanish, from one of these representatives:

• Samantha Kinney | Mendocino County Health and Human Services Agency

The coalition will use the findings of the anonymous survey, along with information gathered through focus groups, a key informant survey, and publicly available data to develop a Community Health Improvement Plan that prioritizes the health needs of Mendocino County residents and sets goals and short-and long-term strategies to achieve them.

MCC Earns National Recognition for Patient-Centered Care

NCQA Patient-Centered Medical Home™ standards emphasize enhanced care through patient-clinician partnership

WASHINGTON, DC— On June 24, 2015, The National Committee for Quality Assurance (NCQA) announced that Mendocino Coast Clinics of Fort Bragg, California has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long]term, participative relationships. 

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that Mendocino Coast Clinics has the tools, systems and resources to provide its patients with the right care, at the right time.”

To earn recognition, which is valid for three years, Mendocino Coast Clinics demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

Help Put Leaves on Our Dental Tree!

Our Dental Tree has deep roots. It started out as a Halloween decoration, and then it was decorated for the holiday season. With the new year came a new idea from our dental assistant Carol Linville. What if each leaf on the tree represented a child who came in for a dental check-up or a fluoride treatment? The tree grew more beautiful as more and more children’s smiles became more and more beautiful!

We keep adding new leaves every day, and seeing the tree makes everyone smile. Why not bring your child in to have a dental check-up and add his or her very own leaf to the tree? We’d love to see you!
